What are some of the day-to-day duties?
The Acute Coordination Partner is responsible for coordinating and tracking field operations for our Medical Technician Partners (MTPs) as they deliver urgent and routine in-home medical care to seniors. This individual plays a key role in ensuring timely response to urgent needs, proactive outreach to schedule home visits, and optimization of field resources. Additionally, this partner is responsible for scheduling patient visits after they are discharged home from Emergency hospital visits.
The ideal candidate has dispatch experience, excellent communication skills, strong situational awareness, and thrives in a fast-paced, mission-driven environment
The Acute Coordination Partner day-to-day duties will include but are not limited to:
· Serve as central point of contact for incoming requests for MTP visits
· Dispatch Medical Technician Partners (MTPs) to urgent same-day appointments in coordination with Acute Care Team and Center Operations
· Monitor real-time availability and location of MTPs and to assign the most appropriate responder
· Assess MTP schedules for the upcoming 24-48 hrs and arrange visits to account for geographical efficiency and to provide availability for urgent visit needs
· Ensure Medical Technician Partners are scheduled at capacity
· Proactively call patients to schedule for home visits according to ideal visit cadence
· Communicate with patients around expected arrival times, appointment changes and expectations
· Schedule follow-up appointments for patients discharged home from an Emergency Department visit.
· Assist in monitoring missed appointments for patients discharged home from the hospital to ensure visits and transportation are scheduled urgently.
· Collaborate with Clinical Leadership, Acute Care Team, Operations Leaders, and Medical Technician Partners to coordinate daily patient needs and to support process improvement initiatives.
· Providing support to colleagues as needed in a team-based environment to ensure our patients receive the highest quality of service and care possible
· Additional duties as assigned
What are the requirements for this position?
· Ability to work for any employer in the United States
· Minimum of two years of experience with real-time resource deployment
· Exceptional phone communication and customer service skills
· High school diploma or G.E.D.
· Strong interpersonal skills with experience working in settings requiring frequent communication via phone and in-person with patient and care team members
· Experience with basic office tools such as email, Outlook calendar, etc.
· Strong verbal, written, and reading comprehension skills in English
· Ability to work regular office hours Monday through Friday
What are the “preferred, nice-to-haves”, for this position?
· Experience with Athena or another Electronic Health Record software
· Experience with seniors, under-served populations, individuals with disabilities or other complex social needs
· Bilingual in Haitian Creole, Spanish, Mandarin, Cantonese, or Vietnamese

Monday – Friday during standard business hours.
