Role Purpose
The Practice Engagement Manager (PEM) is the on-the-ground face of Connie Health within an assigned geographic territory. This role is the critical link between Connie Health and our partner medical practices. The PEM builds trusted relationships with practice leadership and staff, develops and executes tailored engagement plans, and drives Medicare enrollment activity that benefits patients, practices, and Connie Health alike.
This is a field-first role requiring regular in-person visits to partner practices. The ideal candidate combines the energy of a sales professional with the credibility of a healthcare educator and the discipline of an account manager.
Practice Relationship Management
- Serve as the primary day-to-day point of contact for assigned partner practices within the territory
- Build deep, trusted relationships with physicians, practice administrators, front office staff, and clinical teams
- Conduct regular in-person visits to partner practices to assess engagement levels, address issues, and strengthen relationships
- Understand each practice’s business model, patient demographics, and operational dynamics to customize the Connie Health approach
Engagement Planning & Execution
- Develop individualized engagement plans for each partner practice based on practice size, Medicare patient volume, and staff receptiveness
- Assess and prioritize practices by their potential to generate Medicare enrollments, ensuring efficient use of time and resources
- Monitor plan execution against defined milestones and enrollment targets, adjusting tactics when needed
- Ensure each practice is equipped with the right educational materials in lobbies, front offices, and patient-facing touch points
Medicare Education & Patient Outreach
- Educate practice staff on Medicare basics – Original Medicare, Medicare Supplement, and Medicare Advantage – including the pros, cons, and key differences
- Train practice teams on the Connie Health referral process and how to introduce Connie to Medicare-eligible patients
- Maintain a year-round Medicare engagement calendar with clear practice-level priorities for each key period: Annual Enrollment Period (Q4), Open Enrollment (Q1), and monthly T65 outreach. Ensure every partner practice understands the playbook for each window and is executing on time.
- Conduct monthly webinars on behalf of partner practices for patients turning 65. Drive practice accountability for sending outbound communications — email and text — to their T65 patient population ahead of each session.
- Remain current on Medicare regulatory changes, plan updates, and market shifts; share relevant intelligence proactively with practices
Intelligence & Reporting
- Gather and communicate competitive intelligence, local market dynamics, and practice-level insights to Connie Health leadership
- Track and report engagement metrics, referral activity, and enrollment outcomes for each assigned practice
- Identify barriers to enrollment and escalate systemic issues with proposed solutions
- Maintain accurate records of practice visits, conversations, and follow-up commitments in Connie Health’s systems
Minimum Qualifications
- 3+ years of experience in healthcare sales, provider relations, account management, practice operations, or a related healthcare-facing role
- Experience building relationships with physician practices, healthcare providers, or healthcare organizations
- Strong communication, presentation, and organizational skills
- Ability to work independently in a field-based environment and travel extensively within an assigned territory
- Valid driver’s license and reliable transportation
Preferred Qualifications
- 5+ years of experience in Medicare, provider engagement, healthcare partnerships, or healthcare account management
- Knowledge of Medicare Advantage, Medicare Supplement, and Part D plans
- Experience working with primary care practices, health plans, or value-based care organizations
- Experience conducting educational presentations, webinars, or training sessions
- Active health insurance license or ability to obtain one
Knowledge & Experience
- Solid working knowledge of Medicare and Medicare plan options
- Familiarity with physician practice operations and healthcare delivery models
- Knowledge of community resources and support services available to Medicare-age patients
